Data gathered by Coppingford Chiropractic is stored securely on PracticeHub servers, accessible only by the practitioner and kept under password protection. In order to provide safe and effective health care services, we have a legitimate interest in collecting personal information about your health and wellbeing, to fulfil our legal obligations and, if necessary, to protect your vital interests. Your consent will be needed in order for us to collect this data. This may include:

  • name, address, phone number, date of birth and marital status;
  • the name of your next of kin, family members or of any person named by you;
  • the name, address and telephone number of your general practitioner;

  • a record of any complaints/compliments made by you and the action taken in respect of any such complaint/compliment;
  • any referrals or communications with other health care providers;
  • banking or payment details, such as invoices or receipts.

If this data is not provided, we will be unable to carry out chiropractic treatment.

You may of course request to see your patient records free of charge, or ask to have them transferred to another practitioner in the event you move to a different clinic. 

With your consent, this data may also be shared with medical third parties to refer you to another health care professional, for further health investigation, or to update your GP on your progress. We may also send you correspondence and communications such as appointment reminders, offers, and surveys to keep track of your progress or improve our services.

We have a legal obligation to hold your patient record for 8 years following your last appointment, or until you turn 25 if this is longer. After this point, you are welcome to ask to have your file erased.

We have a legal obligation to use your personal information in the event it is necessary for us to protect life or health – for example contacting police, social services, or emergency services in the event of emergency, if a child or young person is deemed to be at risk of harm (Children Act 2004 and Working Together to Safeguard Children), or if you are deemed to be a risk to yourself or others.

You have a right to revoke your consent at any time.
